Nate West
President & Executive Director
Nate West was born and raised in Phoenix, Arizona, and is firmly rooted in the local community. A proven business leader, Nate owns and operates multiple local businesses, and he takes pride in providing valuable services to his community while creating meaningful employment opportunities.
He holds both a Finance degree and an MBA from Arizona State University. As a Marine Corps Officer, Nate deployed to the Middle East, leading over a hundred Marines and honing his leadership and resilience — qualities he tries to bring to every aspect of his work today. Heritage Home Care proudly operates as a Service-Disabled Veteran-Owned Small Business under his leadership.
Nate’s decision to enter the caregiving industry was deeply personal. Having witnessed the immense effort and dedication his mother put into caring for her own mother, he saw the emotional and physical challenges families face when tasked with providing care. He believes everyone deserves the choice to stay in the comfort of their own home, and he is passionate about making that a reality through Heritage Home Care.
Outside of work, Nate volunteers with the Salvation Army, Arizona Friends of Foster Children Foundation, and Big Brothers Big Sisters. A sports enthusiast, he supports all the local Phoenix teams and loves spending time with his wife, Becky, and their two dogs.
Justin Score
Vice President & Director of Business Development
Justin Score serves as the Vice President of Heritage Home Care, leveraging over 15 years of experience in the medical industry. He holds a degree in Health Sciences from Arizona State University, which forms the foundation of his expertise in healthcare management and senior care.
Justin’s passion for senior care stems from personal experiences with his aunt and uncle, both of whom faced the challenges of dementia. Their journey inspired his commitment to providing compassionate, high-quality care to every client at Heritage Home Care. Justin is dedicated to fostering trust and ensuring each client receives personalized attention tailored to their needs.
Outside of work, Justin enjoys spending time with his family, traveling to new destinations, and playing golf. He is also a proud dog owner to Fitzgerald and cherishes moments with his son, Mike. Justin’s dedication to improving the lives of seniors guides his leadership and vision for Heritage Home Care.
Emily Tao
Director of Community Relations
Emily Tao brings a wealth of experience in senior care, with a deep-rooted passion for person-centered support. Starting her career as a certified caregiver and house manager in assisted living group homes, Emily developed hands-on expertise in providing compassionate, individualized care. Recognizing the opportunity to extend her impact, she transitioned into in-home care, where she could help individuals remain comfortable and supported in their own homes.
Over the years, Emily has honed her skills in recruiting, care management, and operations, enabling her to develop a comprehensive understanding of the senior care continuum. Now, as Director of Community Relations, Emily focuses on building meaningful partnerships with organizations that share her philosophy of empowering seniors through education, resources, and personalized care solutions.
Emily takes pride in serving as a trusted advocate for seniors and their families. She is dedicated to connecting them with valuable resources while fostering relationships that enhance the well-being of those in need.
The youngest of seven children, Emily values family deeply and enjoys spending her free time with loved ones and her two dogs, Bailey and One-Eyed Willie.
